The George House Trust has contributed a copy of their volunteer application form to the Good Practice Bank.

Please note: please bear in mind that this application form has been created for a specific organisation in a specific context, and should therefore be used as a guide rather than adopted wholesale. For further information, please visit our 'top tips on using the Good Practice Bank'.
We like this application form because it is clear and easy to read. The questions have been typed in a large bold font. The boxes on the form will help to guide the person completing it.
The contact details for the volunteer coordinator are given at the bottom of the form, and potential volunteers are invited to call directly or do their own research on the website.
It is a good idea to ask how the potential volunteer heard about your organisation, because it will help you identify what works and plan future recruitment.
It's good to point out at an early stage if Criminal Record Bureau checks will be required. It's also important to note that they are only required for certain roles.
